Las obsesiones del poder panóptico

El poder vive obsesionado. Todos tienen la obsesión del encierro, del control, de la vigilancia, de la norma, de la disciplina. Y en esto son acompañados, como siempre ha sido en la historia, por los médicos para normalizar a los deviants. Muy bien lo han explicado los filósofos franceses Foucault y Deleuze y el sociólogo Pierre Bourdieu.

El panóptico diseñado por Bentham es según Foucault el ojo supremo del poder.

   El presidente Iván Duque y su ministro de Salud están obsesionados con encerrar a los mayores de 70 años y la alcaldesa de Bogotá Claudia López está desesperada por encerrar a toda Bogotá. La teoría detrás de esto es que hay que castigar y vigilar a los que se desvían de la norma que imponen los epidemiólogos, los consejeros del poder.

   Y esto lo siguen todos los alcaldes, administradores de centros comerciales con sus normas ridículas, y los administradores de juntas administrativas. Y el poder político, sea de izquierda, derecha o centro, no hace nada, se queda callado o asiente, sin ninguna discusión al poder.

   La pandemia del coronavirus se ha tratado como si fuera solamente un problema médico, es decir, se ha medicalizado, como lo hicieron antes los siquiatras con los enfermos mentales, con los prisioneros, con los homosexuales, los trans, con las lesbianas, con los niños, en general, con todos los que se desvían de las normas impuestas por el poder.

  Esta pandemia no se puede controlar en Colombia si no se adopta un enfoque global, especialmente si no se entiende que este es también un problema social y económico.

Encerrar a los adultos

  El reaccionario y ultra neoliberal gobierno de Iván Duque anda loco por haber perdido una tutela que presentaron unos «abuelitos», según su expresión humillante, y ha anunciado que va a responder a la misma.

    Hay siete argumentos para refutar las teorías del panóptico de Duque, su ministro de salud y los consejeros del Imperial College, especialmente de su experto Neil Ferguson, partidario acérrimo del encerramiento, en especial de los adultos, y de las privatizaciones en el sector salud.

    Primero, Colombia es uno de los pocos, si no el único país del mundo que ha adoptado esta política. Hubo intentos en Francia, Alemania e Inglaterra de adoptar esta política absurda, pero el rechazo fue tan generalizado que los gobiernos dieron marcha atrás.

   Segundo, desde un punto de vista económico, vale más la vida de un joven que la de un adulto, porque la vida productiva es mayor, así que a los que hay que cuidar más es a los de menos de 30 años. Es más peligroso que los jóvenes prendan el covid-19 a los adultos que lo contrario. De ahí que en varios países la idea es la de hacer testeos a los menores de 30 años, porque la mayoría son asintomáticos y con signos muy leves. Y si visitan a sus padres y abuelitos, les prenden el virus.

     Tercero, el poder manipula las estadísticas. O no las entiende o lo hace adrede. Históricamente, la probabilidad de muerte es mayor entre más edad usted tenga. Si se analizan las cifras de muertos de Colombia en los últimos diez años, se encuentra que los mayores de 70 años está más del 50% de los muertos. En artículo de El Espectador del jueves 9 de julio, José Fernando Isaza lo mostró con estadísticas, claramente. A medida que se entra a más de 70 años, la probabilidad de morir por enfermedades es alta. Con la edad vienen enfermedades como hipertensión, diabetes, problemas cardiovasculares, pulmonares, renales, digestivas, en fin, todo tipo de patologías.  Para mayores de 80 años la situación es más patética.

   Cuarto, la edad promedio de vida viene aumentando en Colombia, y ya muchas personas de los 70 años trabajan para sostenerse o porque se consideran aún productivos para la sociedad. Encerrarlos es romperles un modo de vida que es normal en las sociedades modernas, es ir contra sus derechos como ciudadanos, y es una discriminación absurda, propia de dictadores.

   Quinto, los mayores de 70 años mueren por viejos, no solo por portar el coronavirus, mueren porque tienen varias comorbilidades, que es lo normal a medida que usted pasa de 70 u 80 años. Pero, también mueren los jóvenes que tienen comorbilidades, o las personas de 50 o 60 años. Lo que ocurre es que las comorbilidades se dan más a medida que pasan los años. Pero los adultos que no tienen comorbilidades superan el coronavirus.

   El boletín técnico del DANE del 25 de junio de 2020 sobre estadísticas vitales muestra que el número de defunciones no letales según grupos de edad, el 55,3% correspondió a los mayores de 70 años en el primer trimestre de 2020. Igualmente, de la estructura de edades de la población colombiana, para 2018, el 5,4% correspondía a mayores de 70 años. Para Bogotá, de las estadísticas de la Secretaría de Salud, del 9 de julio, el 6,55% de los contagiados y el 35,66% de los fallecidos por el Covid-19 eran de personas de 70 años y más. Para el país, el porcentaje de fallecidos por el virus está alrededor de 55,0%. O sea, los contagios y muertes son un problema de edad, que representa lo que ocurre a nivel de estructura poblacional y muertes por diversas causas no fetales.

  En el primer trimestre hubo 21.365 fallecidos por diferentes patologías, es decir, mucho más que por el coronavirus que al final del primer trimestre no pasaban de 200. Por enfermedades isquémicas del corazón murieron 9.413 colombianos, por enfermedades de las vías respiratorias inferiores 3.657, y así sucesivamente. A 9 de julio se habían presentado 4.714 muertos por el coronavirus.  De los mayores de 70 años, por Covid-19, en Bogotá se tenían 499 fallecidos y a nivel nacional unos 2000. No hay estadísticas todavía, pero es de suponer que para el segundo trimestre murieron otras 22.000 personas por enfermedades no letales diferente al Covid-19.

   Aquí tenemos otra confusión sobre las tasas de mortalidad. La verdadera tasa es la que se calcula sobre todo tipo de contagiados, o sea, incluyendo a los asintomáticos y a las personas con síntomas leves. Aunque no hay estudios empíricos aún serios, se supone que por lo menos el total de contagiados es diez veces más el de las cifras oficiales. En esta forma, las tasas de mortalidad efectivas son inferiores a 0,3% a nivel global, variando, por supuesto por grupos de edades. Entonces para los de mayores de 70 años se reducen a menos 2,0%.

   Sexto, la solución para superar el coronavirus no está en el encierro panóptico, sino en el autocuidado personal, en el control a las cárceles, ancianatos, plazas de mercado, en el distanciamiento social, en evitar aglomeraciones, en alimentarse bien, en tener buena salud, en testear masivamente, comenzando por los jóvenes, en focalizarse donde aparecen brotes de la epidemia, no en localidades enteras sino en los barrios o sitios donde se identifican brotes. Además, esto hace más fácil y eficiente el control Y, sobre todo, en tener un buen sistema médico, que esto es lo que evita las muertes, no el encerramiento.  En tener hospitales modernos con todas las herramientas, equipos, especialistas, salarios bien pagos para el personal de la salud. ¿Cómo se espera controlar una epidemia cuando los resultados de las pruebas se demoran de 11 a 15 días? Imposible. Esto desde un punto de vista médico.

   Séptimo, hay que tener una política social y económica. No es lo mismo Colombia que Italia, Francia, Alemania o Finlandia, todavía con buenos sistemas de bienestar, a pesar del ataque de los neoliberales, donde los desempleados tienen su seguro, hay educación gratuita, salud pública, ayudas de todo tipo, créditos sin tanto problema como en Colombia. Por lo tanto, la gente no tiene necesidad de rebuscarse la vida, de salir a la calle para conseguir el dinero para la comida diaria. Y aquí hay mucha gente de más de 70 años en esta situación en Colombia. En los barrios marginales de las grandes ciudades, hay hacinamiento, pobreza, falta de educación, barrios donde no hay acueducto, hambre, niños que no pueden alimentarse bien. Encerrarlos es imposible y sí se puede, es condenar a millones al hambre, a la desnutrición, a la violencia, a menos que existiera una política de ayudas económicas, un ingreso mínimo, como se ha propuesto, de por lo menos 500 000 pesos por persona. En las grandes urbes, con sus cinturones de miseria no se pueden controlar estos millones de compatriotas. Como se decía en la colonia, la ley se obedece, pero no se cumple.

  Entonces, en estos casos, en un país como Colombia y casi todos los de Latinoamérica, si no hay respuestas sociales y económicas, la solución médica-epidemiológica falla. Son soluciones de corto plazo. Los médicos y los epidemiológicos no tienen el conocimiento de los aspectos sociales y económicos, se concentran en su especialidad, no ven el bosque, no son generalistas, son especialistas que no salen de su enfoque parcial, son reduccionistas.

   Mi teoría es que el problema no es únicamente médico, que solo pensar en el encierro no resuelve el problema. Habría que encerrar obligatoriamente a la población por meses. Además, es imposible controlar a miles de miles de informales, pobres y vulnerables. Se controla, posiblemente, por un tiempo corto, pero después vuelve a surgir el problema.  Pienso, que la demora en producirse el pico de la pandemia en Colombia y varios países de Latinoamérica, es mi hipótesis, tiene que ver con las condiciones de explotación, exclusión, ignorancia, pobreza, desigualdades y falta de bienestar en todo sentido que es norma en este continente, que lleva a lo que el poder llama despectivamente indisciplina social, sin entender por qué esto ocurre.

¿Cuándo llegaremos al quiebre?

Según la experiencia internacional, entre dos y tres meses se llega al quiebre de la pandemia. Esto ha ocurrido en casi todos los países.  En Colombia vamos a cumplir cuatro meses desde que se decretó la cuarentena en Bogotá y todavía siguen creciendo los infectados en las regiones más importantes (Bogotá, Atlántico, Antioquia, Valle del Cauca y Bolívar). Predecir más de diez días es imposible en un ambiente tan complejo, en el sentido técnico del término, en que nos encontramos. Ya lo he probado en varios artículos y en mis propias proyecciones con los mejores métodos estadísticos.

    Todas las proyecciones de los epidemiólogos han fallado. Es difícil predecir hasta que va a pasar al día siguiente. Nuestras proyecciones no van más allá de diez días donde hemos probado que son bastante exactas. Rompiendo esta regla, proyectamos hasta el 19 de julio, y los resultados para el país nos dicen que seguiría una tendencia creciente de infectados, con unos promedios diarios que van de 3542 a 4378 casos, según la técnica utilizada, todo con base en la información que teníamos hasta el 28 de junio. Para Bogotá, la banda de predicción estaría entre 1100 a 1519 casos diarios. Pero, dada la nueva información del mes de julio estas proyecciones deben cambiar hacia arriba.

    Es de esperar, pero es solamente una hipótesis, que el quiebre debe ocurrir en julio o agosto. Pero en Colombia, país del mundo en que todo es excepción, no se puede afirmar nada con seguridad.

   En conclusión, señores del poder, presidente, ministros, alcaldes, gobernadores, a pensar muy bien, estamos en Colombia, no en Alemania, el problema no es solamente médico, es más complejo, tiene varias aristas  adicionales: económicas, sociales, mentales, territoriales, de pobreza, de exclusión, de marginalidad; de mal sistema de salud, de EPS ineficientes, de una salud privada para minorías, de hospitales públicos acabados, robados, destruidos, de médicos, enfermeros y especialistas mal pagados, cuando les pagan; de mal sistema de acueducto en barros marginales, especialmente en la Costa Atlántica; de corrupción porque se han robado los recursos de la salud en el pasado y aún en el presente;  de políticas neoliberales que se olvidaron de la salud pública; de un  estado narco paramilitar; de militares que violan a niñas, de muertes constantes de líderes sociales y de ex miembros de las Farc; de un autoritarismo creciente y sin ningún control político; de un  estado que ha aprovechado la coyuntura para imponer por decretos lo que no pudo hacer antes por la protesta ciudadana.

    En fin, el problema, señor presidente, ministro de salud y alcaldesa Claudia López, no se resuelve simplemente con encierros, con volver a Colombia, y en especial a Bogotá, un panóptico.
